Investigation Summary
Employee #1, of the San Francisco Water Department, was in a 15 ft deep by 10 ft long by 10 ft wide trench. He was repairing and replacing a 52 ft long section of water line that had ruptured, when the west wall of the trench caved in. Employee #1was buried him for approximately 8 to 10 seconds.
