Employee suffers seizure and drowns in fountain

At approximately 7:30 a.m. on October 3, 1990, Employee #1 was picking leaves off the water intake screen of a water fountain. The water fountain was approximately 2 ft deep. The employee had a seizure, collapsed into the fountain, and drowned.
