Investigation Summary
At approximately 8:05 a.m. on November 19, 1991, Paper Cal Steel Company had just recoiled about 30 to 32 ft of 3/8 in. wide, A572, GR 50 steel ribbon wound coil on the Herr-Voss dual cone type uncoiler. Employee #1 had installed two 1/4 in. by .031 high tensile steel strapping with pusher-type 2 1/4 in. long seals (double upcut), on each steel band. The employee went over to the control panel, approximately 25 ft away, lifted the hold down and lowered the coil car, returned to the recoiled ribbon wound coil, and was placing the third steel strap into place when the recoiled ribbon wound coil let loose. The employee was found against the west wall. The cause of death was multiple blunt force traumatic injuries. There were no witnesses. It appears from the injuries that the employee was hit in the right chest/rib area by a blunt object, either the steel when it uncoiled or an object he hit after being thrown from the uncoiling steel.
