EMPLOYEE STANDING IN MOVING TRUCK BED STRUCK BRIDGE OVERPASS

EMLOYEE #1 WAS RIDING IN A STANDING POSITION ON A TRUCK LOADED WITH STRAW BALES. THE HEIGHT OF THE UPPERMOST BALES WAS APPROXIMATELY 12 FEET ABOVE GROUND LEVEL. THE TRUCK PASSED UNDER AN OVERPASS WHILE THE EMPLOYEE WAS STANDING ON THE BALES. THE EMPLOYEE STRUCK THE BRIDGE AT APPROXIMATELY 20 MPH WITH HIS UPPER BACK AND HEAD. THE EMPLOYEE WAS LOOKING AT THE TRAILING TRAFFIC AND WAS UNAWARE THAT THE TRUCK WAS APPROACHING THE OVERPASS. THE EMPLOYEE DIED FROM HEAD INJURIES SUSTAINED. THE OVERPASS HAD A 16 FOOT CLEARANCE FROM GROUND LEVEL. THE ACCIDENT OCCURRED AT 5:30 PM ON OCTOBER 3RD, 1991.
