Employee injured in fall from ladder on scaffold

Employee #1 pushed a rolling scaffold against the wall, locked the wheels, put a 6 ft stepladder on top of the scaffold, and climbed on it to skim coat a butt joint. The rolling scaffold fell outward, away from the wall. Consequently, the stepladder fell with the employee as he landed on his rear, fell over backward, and struck his head against the concrete floor. The employee was transported to Evergreen Medical Center for treatment and released the same day under caution and advice to seek further treatment. After a week's rest and another two weeks of physical therapy, the employee seemed able to continue work with no physical limitations.
