Worker is caught by Rotating Shaft and Dies

At 8:15 a.m. on January 6, 2016, a worker was cleaning the area around a grease fitting associated with a drive shaft, when his jacket became caught on the collar bolts that were affixed to the rotating shaft. The worker was pulled into the machinery, suffering multiple contusions to his upper body, which resulted in his death.
