Investigation Summary
Employee #1 was felling a tree in which a 1 1/2 in. diameter by 12 ft long limb had apparently become caught. When the tree started to fall, the limb shook loose and fell. Its sharp end struck Employee #1 on the upper left side of his chest, piercing his heart. He was killed.
