Employee breaks both arms in fall from roof

Employee #1 was on a roof and above an area that had just been cleaned with a water pressure device. A coworker handed up the wand to Employee #1, who, after starting to clean the roof, slipped and fell approximately 35 feet, fracturing both arms and going into convulsions. There were fall-protection devices at the site; however, none were used. In addition, no fall-protection work plan had been developed for this job site, nor had the employees been trained.
