Employee injured while adjusting clipper saw

Employee #1, a millwright, was tightening a coupling on a shingle saw. The clipper saw blade and the saw arbor had not come to a complete stop when Employee #1 reached into the machine to perform this task. His left arm was cut and broken. No lockout/tagout program or written safety program had been developed by the employer.
