Employee Killed When The Brakes Failed And Water Truck Over

At 12:30 p.m. on January 9, 2012, Employee #1 had just completed loading his tru ck with approximately 70 barrels of waste water from a gas well. After leaving t he gas well site, Employee #1 was traveling in his water truck on a private roa d. Employee #1 truck overturned approximately one mile from the gas well site. The first half mile was a steep grade and the last half mile was leveled off. A Colorado State Patrol Level 1 Investigator stated that the rear brakes on both a xles failed resulting in excessive speed. The water truck rolled multiple times before it came to rest on its wheels. Employee #1 was wearing his seat belt and shoulder harness. The truck roof crashed down on to Employee#1 and he was kille d from blunt trauma to the upper body.
