Investigation Summary
Employee #1 was cutting rebar with an acetylene torch and tying curtain to the concrete form. The employee fell 22 to 25 ft onto the concrete footing. There was no evidence of safety equipment failure or broken ties on the rebar curtain. Employee #1 was wearing a safety belt and an iron worker's lanyard instead of a full body harness and double lanyard for 100 percent tie-off.
