Two employees injured in fall from screen of paper machine

Employees #1 and #2 were attempting to tie up a 3 in. hose that was bouncing on top of a 1 degree screen in the basement of a #20 paper machine. When they lifted the hose, the cinch clips gave way, causing the hose to detach. Slurry from the machine came out at 30 psi, hitting both employees in the chest. Employee #2 went head first over the edge, but righted himself when holding onto the sling. Employee #1 was pushed into some pipes, then fell backwards 12 ft over the pipes. Both employees suffered contusions to their backs and legs. The fall could have been prevented if fall arrest or fall protection had been used and/or if the machine had been locked out.
