Investigation Summary
An employee was pushing a particle board off forklift forks into a bin. Employee #1 saw that he was having trouble and grabbed a 23-inch stool to help him push from the outside end. Employee #1 pushed the particle board into the bin and when he went to stand back up, he went over backward and hit his head on the concrete floor. He died from his injury.
