Employee killed after slipping onto debarker conveyor

At about 7:25 p.m. on January 18, 1993, Employee #1 was alone operating a mechanical de-barking machine at the Loth Industries Mill, although he works for F.A. Koenig & Sons, a company working a portion of the mill at night. Employee #1 had no more logs on the infeed conveyor and climbed onto the framework of the conveyor to see if he could view how many logs were left. A coworker in the loading booth came out and held up two fingers, meaning that two logs were left. Employee #1 either stepped or slipped onto the running conveyor, which moved him feet first into the debarker. Employee #1 was killed, suffering extensive crushing injuries and amputation of the legs and lower torso.
