Investigation Summary
At approximately 3:30 a.m. on May 30, 1995, Employee #1 was working on an STP web forming machine that manufactured a nonwoven polypropylene material. He was removing residue from the unguarded #2 compression roller by hand using a rag soaked with isopropyl alcohol while the machine was running. His right hand was pulled into the in-going nip point, followed by his body. Employee #1 was killed. He had been working from a platform that exposed him to the in-going nip point. The belt and compression roller were located 36 1/2 in. above the work platform. The #2 compression roller weighed 1,690 lb and the conveyor belt ran 1,380 ft per minute.
