Employee killed when struck in chest by log

At approximately 12:52 p.m. on July 28, 1994, Employee #1 was working as a driver/helper for a self-employed, part-time logger. Employee #1 was with the logger/owner when he drove the company's flat-bed Ford truck to a local sawmill. The load of approximately 12 to 14 ft long tulip poplar logs was stacked about 1 ft above the top of the 44 in. tall metal truck standards and was partially secured with a single chain-binder that was placed approximately mid-load. Employee #1 exited the truck and proceeded to unlatch the chain-binder (which was on the driver's side), without first securing the logs with unloading lines or other unloading devices. Immediately, a log weighing approximately 500 lb dislodged and fell. It struck Employee #1 in the chest area. Employee #1 died from a severe crush injury to the chest. There were no witnesses to the actual falling or striking of the log.
