Employee killed when pinned by gate

Employee #1 was closing a manually-operated chain link fence gate when the gate fell forward off its supporting pillar and onto him. The gate pinned him by the throat against the control box for an automatic gate. Employee #1 was unable to lift the gate from his throat and apparently suffocated. He was killed.
