Investigation Summary
Two city employees were installing a sewer tap in a trench approximately 35 feet long and 6 to 8 feet deep. Water was coming into the trench and creating mud. The employees were pushing the mud away from the area where they were going to install the tap. One employee noticed that the walls were beginning to spall and said, "We need to get out of here." He turned and went up the ladder. As Employee #1 turned toward the ladder, the trench caved in. He was covered over his head up to 1 to 2 feet. The employee who successfully exited the trench went for help. Employee #1, who was buried for approximately 35 minutes, died.
