Employee killed by falling residue on vessel walls

Employee #1 was the lead man of a three-man crew who were cleaning a fluidized bed vessel. The crew was using a round point shovel and hand-held pick to break off pieces of the residue from the vessel's wall. One crew man was standing on or near the top of a 40 ft ladder, while another was holding the ladder's base. Employee #1 was standing near the southern wall using a steel rod to break up pieces of residue. A large piece of residue pulled loose from the wall, fell on the employee, and pinned him to the bottom of the vessel from the waist down. He suffered multiple fractures and died 6 days later. The vessel is approximately 50 ft high and approximately 20 ft in diameter.
