Employees injured during hydrogen gas explosion

At approximately 7:45 a.m. on June 12, 1992, a leak was discovered at the brine exit flange on the #24 sodium chlorate electrolytic cell. The cell line was shut down at 8:01 a.m. At this time, a purge cycle to remove toxic/flammable gases from the header system was initiated. This included 15 CFM of nitrogen for 15 minutes, a 30 minute rest period, and an additional 15 CFM of nitrogen for 15 minutes. Employees thought that the first purge was successful due to the audible purging sound and the readings of the rotameters on the nitrogen line. An explosion occurred at 8:49 a.m., about the time the second purge was to begin. Employee #1 was walking through the area as a short-cut. Employee #2 was at the base of the #24 cell and was hit by flying objects. Employees #3 and #4 were on the catwalk near the #24 and #26 cells, respectively. The four employees sustained various injuries, but were not hospitalized.
