Investigation Summary
Employee #1 was working on a drop sorter at a lumber mill. The drop sorter has a series of holes through which lumber is dropped according to length and width. Occasionally Employee #1 had to stop the drop sorter and step onto the platform to straighten a misaligned board. Apparently, when Employee #1 stepped on the drop sorter platform, he slipped and fell headfirst through a hole in the platform. He fell approximately 15 feet, striking a steel beam, then landing on the concrete floor. Employee #1 was killed.
