Investigation Summary
A fabric worker was working on a Sulzer Ruti Terry Cloth Wearing Machine. During this operation, a Loom Take Up Machine attached to the first machine malfunctioned. The employee examined the machinery and determined that a chain drive had jumped its sprokets. He took the machine out of service and tried to fix the chain drive. The employee's supervisor came over and tried to assist, as did some coworkers. The employees decided to remove the lockout and jog the motor to get the chain back in place. The employee who had been working at the machine tried to hand feed the chain on its sproket. During the attempt, he contacted an exposed energized part on a 480-volt transformer and was electrocuted.
