Employee killed when struck on head by truck

At approximately 5:40 a.m. on September 20, 1990, Employee #1 was removing a materials handling insertion truck from a Vamatex loom after loading the beam. Backing up, Employee #1 stepped into a 7 in. concrete trench. He lost his balance and fell to the floor. The materials handling insertion truck, under momentum, also entered the trench and fell onto Employee #1. Employee #1 was struck on the head and killed instantly.
