Investigation Summary
At approximately 2:20 p.m. on February 25, 1991, Employee #1 was installing an expansion joint in the boiler room piping from a four-sided wooden shipping crate elevated 25 ft by a forklift. When Employee #1 stepped to one side of the wooden crate/pallet to say something to his supervisor, the crate flipped over, causing the crate and the employee to fall to the gravel. He was killed when his head and face struck the gravel. The forks of the forklift were placed under the pallet in such a way that the pallet's bottom railings were not under the fork of the lift.
