Employee dies after caught in fire inside condenser

Employee #1 went into the condenser to unstop it. The opening was 1 ft by 2 in. and was 11 ft, 4 in. from the floor. An unknown source of ignition caused a flash fire of the flammable fibers in the condenser. Employee #1 sustained burns to the face, arms, and legs. He died from acute hypoxia due to bilateral pulmonary embolus.
