Investigation Summary
Employee #1 and a coworker of the George Kountoppes Painting Co., were working from a swing-stage scaffold approximately 35 feet high, moving sections of bridge lattice. The swing stage was supported by two job-built rollers that rode on the 16 inch wide bridge beams. As they moved the scaffold, the roller suspension system became disengaged from the beam, causing the scaffold and Employee #1 to fall to the road. Employee #1 was killed. The accident happened in Wayne County, MI, area 1. The employer was issued five serious citations.
