AN EMPLOYEE WAS ELECTROCUTED AS HE UNPLUGGED AN EXTENSION CORD SET SUPPLYING POWER TO A 120-VOLT CEMENT VIBRATOR. THE CORD SET WAS HAND MADE AND WAS NOT APPROVED FOR WET LOCATIONS.
