Investigation Summary
On April 5, 1995, Employee #1 of Tree Top Sylvan Resort, was trimming trees by the 18th green of a golf course with two other employees. He was standing on the uphill side of the tree being cut and it fell on top of him. The employee received severe head injuries and died.
