Investigation Summary
On July 8, 1990, Employees #1 and #2 were cleaning the exterior of a building from a swing stage scaffold at the eleventh floor. The scaffold was overloaded and failed. Both employees and the scaffold fell to the sidewalk. Employees #1 and #2 died.
