Investigation Summary
On September 28, 1990, Employee #1, of Equipment Mfg. Inc., was inspecting a shipment of coiled steel measuring 8 in. wide and 63 in. high using a micrometer. The coils shifted and fell over, pinning the employee between the coils and a steel beam. He died. The coils had been standing upright and unsecured. A serious citation was issued.
