EMPLOYEE #1 WAS ATTEMPTING TO CHECK A COKE MOVING MACHINE AT AN OPERATING BLAST FURNACE. HE WAS EXPOSED TO, AND OVERCOME BY, CARBON MONOXIDE GAS GIVEN OFF BY THE BLAST FURNACE PROCESS.
