EMPLOYEE KILLED WHEN CAUGHT UNDER AUTOMOTIVE LIFT

On March 3, 1990, Employee #1 was working under a single post automotive lift. He was repairing a leak on a vehicle. The lifting mechanism failed and the lift came down on Employee #1. Employee #1 died. There was no auxiliary support device in place to prevent the lift from dropping when the lifting mechanism failed.
