Investigation Summary
On September 21, 1990, Employee #1 and a fork truck driver were moving a 5,000 lb unloader parts dumper with a 4,200 lb capacity HiLo. The unloader was secured to the HiLo mast by a chain. While moving the load, the HiLo operator raised the forks to clear a 48 in. high guard post and the load shifted and fell from the forks. Employee #1 was pinned between the unloader and two stacked parts tubs. He died of severe head trauma. Citations were issued for violations of 2172(1), 2187, 2193 (D), 2193(E), and 3370(1).
