Employee dies after fall from ladder

On December 13, 2003, Employee #1 was carrying a skylight (2 ft by 4 ft and weighing approximately 40 pounds) up an extension ladder to a second-story level. He fell approximately 10 ft to a concrete floor. Employee #1 died several hours later at a hospital of internal injuries.
