Investigation Summary
On April 28, 2004, Employee #1 was working with the owner of a sign company. They had worked numerous times before and were in the process of digging out three holes, approximately 7 ft deep and 30 inches wide. The owner was operating a 1985 Ford digger derrick truck. At the third hole, they hit concrete at 64 inches deep. Employee #1 entered the hole and drilled with a rotating hammer into the concrete to break it up, and exited the hole. The owner saw Employee #1 standing nearby and proceeded to place the auger back in the hole and continued to dig. The owner stated that Employee #1 began walking east of the holes, while he was pulling up and swinging the auger westward then north to spin off the dirt. The owner then swung the auger back to the hole and began digging again. The owner pulled up the auger and saw Employee #1 attached to the auger. The owner couldn't see into the hole from the operator's seat and did not know that Employee #1 had returned to the hole. Employee #1 died.
