Investigation Summary
At 1:00 p.m. on November 10, 2015, Employee #1, employed by a roofing company, was replacing the roof on a flat-roofed commercial building. He was removing roof curbs (unused equipment enclosures) and replacing them with aluminum cover panels. Employee #1 fell through a floor opening that measured 4 feet by 4 feet.
