Employee killed in 20 ft fall from pipe rack

At approximately 4:15 p.m. on February 13, 1991, Employee #1 went up the pipe rack of unit 191 in Complex IV to open the valve of a steam header condensate line. The steam line trap had been repaired at grade or ground level. The employee stepped on a steam tracer line and it broke. He was killed when he fell approximately 20 ft to the concrete floor.
