Employee killed when steel cage collapses

Employee #1 was inside a 47 ft long and 4 ft 6 in. wide reinforced steel cage. He was disconnecting a come-along that was being used to hold the cage in a vertical position. While he was in the west end of the cage, the come-along at the east end gave way, stripping the tie wires on one of the reinforced steel bars. The cage collapsed on Employee #1, killing him instantly.
