EMPLOYEE DIES WHEN HEAD CAUGHT BETWEEN CHAIN AND IRON MEMBER

Employee #1 was attempting to grease the bearings on a log chain conveyor that was being changed-out with a new chain. Apparently Employee #1 reached through the idle chain to grease a fitting when the chain was started and rotated to a position where another 20 ft section could be replaced. As the chain was moving, a 5 in. lug on the chain pulled Employee #1's head into an area 2.5 in. wide, between the machinery and a support beam resulting in severe head and neck injuries. Employee #1 was killed.
