Employee struck and killed by falling bridge truss

At approximately 2:30 p.m. on May 2, 1994, Employee #1 was working as a roustabout cleanup man at a site. He was working in an 8-ft-wide area near a 10-ft-high by 100-ft-long by 29-ft-wide truss that was sitting on three 4-in. by 6-in. wooden blocks. The structure, which weighed approximately 20 tons, had been freestanding in the area for about 6 hours. At approximately 2:30 p.m., coworkers in the area saw the truss fall, or rapidly topple to one side, and pin the Employee #1 across the back by the bridge section. He died of injuries sustained in the accident.
