Investigation Summary
Employee #1 and three coworkers were installing insulation and metal roofing on the top of a boiler building under construction. All four employees had returned from lunch and were stretching and attaching the first piece of insulation for the roof. They were wearing safety harnesses and lanyards and were attached to a life cable going across the apex of the roof. During the stretching/securing process, Employee #1 was working at the tip of the roof and made a shift in his working position. His fellow workers saw him fall backward 75 ft to the concrete. He sustained multiple fractures and was killed. Employee #1 did not have his lanyard attached.
