Employee crushed to death under vehicle

Employee #1 was beneath a car. The two front lift arms were improperly placed. The car tilted forward and fell approximately 3 to 4 feet, front-end first. (This caused the car's rear end to rise 6 to 7 feet.) The force of the vehicle on the front lift arms caused them to collapse backward and crush Employee #1. Employee #1 was untrained in how to use different types of vehicle lifts. He had apparently tried to right the vehicle by pulling down on its rear axle directly underneath the car.
