Employee dies in locomotive accident

Employee #1, a switchman, age 21, and a locomotive operator were assigned to switch rail tank cars. They were using two-way radios for communication. Several switches had been made, leaving one tank car attached to the train engine. Employee #1 told the locomotive operator to come forward after he had thrown the switch. The locomotive operator proceeded forward. After traveling about 150 feet, he asked Employee #1 how close he was to the other cars, but got no answer. He asked once again and got no answer. The operator stopped the engine and got out, but didn't see Employee #1. He walked around the engine and saw Employee #1 along the railroad tracks. Employee #1 died of his injuries.
