EMPLOYEE KILLED FROM FALL OFF OF A PIPE RACK

On January 15, 1990, an employee used a 3-ft-high portable stepladder to gain access to a pipe rack 7.5 feet high. While standing on the rack, he slipped and fell as he attempted to fasten his harness to the safety line. His head struck the concrete floor where he received fatal head injuries from the fall.
