Employee's jaw fractured after trench caves in

At approximately 3:20 p.m. on June 12, 1992, Employee #1 was shoveling along the bottom of a trench that was approximately 2 to 2 1/2 ft wide and 6 ft deep. The walls of the trench were almost vertical and not shored, braced, or sloped. One side of the trench wall caved in and struck the employee on the back, pinned him against the opposite trench wall, and partially covered him with dirt. The employee sustained a fractured jaw and was hospitalized.
