Investigation Summary
Employee #1 and several coworkers were doing demolition work in a refinery, removing components of an old iso cracker process. They removed a furnace, cut it in sections, and left it lying on the ground. The brackets that support the heat exchanger tubes were cut loose and the heat exchanger tubes, still connected, were lying on the bottom of a section of the unit. One of the sections was lying on the back side of the furnace shell at an approximate 30 degree angle. After the employees tried unsuccessfully to shake the section loose with a crane, they determined that the section was stable. When Employee #1 approached the section of tube to choke it off so it could be moved, the section twisted and fell, crushing the employee's head. He was killed.
