STEEL MILL WORKER KILLED WHEN HE WAS RUN OVER BY FORKLIFT

ON A DARK, STORMY NIGHT, EMPLOYEE #1 WAS WALKING ALONG A ROADWAY BETWEEN BUILDINGS AT A STEEL MILL, CARRYING A MACHINE PART. ANOTHER EMPLOYEE WAS DRIVING A LARGE FORKLIFT TRUCK IN THE SAME DIRECTION. THE FORKLIFT WAS BACKING UP (TRAILING THE LOAD) AND THE OPERATOR DID NOT SEE EMPLOYEE #1. THE FORKLIFT OPERATOR FELT A BUMP, LOOKED BACK AND SAW EMPLOYEE #1 LYING IN THE ROAD. HE HAD BEEN RUN OVER AND CRUSHED TO DEATH. THE FORKLIFT'S HEADLIGHTS (BOTH FRONT AND REAR) WERE OPERABLE, AS WAS THE STROBE LIGHT ON TOP OF THE CAB.
