Investigation Summary
A crew of employees had been assigned the task of preparing three stacks of pipes for shipment. The task consisted of stenciling or labeling the pipes and other related work. The pipes were 80-ft-long and weighed approximately 11 tons each. Each stack of pipes was placed on a row of skids. The stacks of pipes consisted of three rows of pipes on top of one another. The bottom row was pinned or clipped and chocked on each end. When the crew started to work on the second stack, the pins or clips were removed from the working end of the stack leaving only the chocks to support the stack of pipes on that side. A front-end loader removed one of the pipes from the stack, and placed it on the skids about 10 ft from the stack so that the crew could do their work on the pipe. There were five members of the crew who were working on or near this pipe. Employee #1 was between the pipe and the stack of pipes when the stack of pipes collapsed. Employee #1 was crushed between pipes from the stack and the one that he was labeling. Employee #1 was killed.
