Investigation Summary
At approximately 1:50 p.m. on February 5, 1994, Employee #1 was operating an excavator. He was widening a retention pond when the ground under his excavator gave way and caused the heavy equipment to slide into the pond. Employee #1 either fell or was attempting to escape from the sliding excavator when it pinned him underneath the cab in the mud and water. He was pronounced dead at the scene by the coroner. The cause of death was a broken neck.
