Employee killed when head crushed between steel pipes

Employee #1 was finish grinding the end bevels of coated steel natural gas transmission pipes in a pipe coating plant. This is the final process before the pipes are picked up by a forklift and taken to storage or placed on a truck transport. After final inspection, the pipes are rolled out on stringers for pickup and the next pipe is rolled to the grinding and inspection point, which is at the rear of the stringers. There was no anti-roll block device on the stringers to prevent the pipes awaiting pickup from bumping into the pipe that Employee #1 was working on. The forklift bumped the pipes on the stringers while Employee #1 had his head in the space between the two pipes. His skull was crushed and he was killed.
